Process for the Transport of Gametes/Embryos to ONE Fertility

If you are not a ONE Fertility patient, you can email infoivf@onefertility.com with this request.
We will require the lab where the gametes/embryos are stored to complete a document outlining details including how the gametes/embryos were frozen, specific thaw instructions, etc.
We will also require specific infectious disease test results at the time of freezing. Depending on the circumstances, there may be additional requirements that are shared at this time. For example, if the gametes are from a donor there would be additional documentation.
Our Laboratory and Clinical teams will review everything and determine if we can accept the gametes/embryos.
Once we determine that we can accept gametes/embryos we will ask for a consent to be completed. You need to be aware that there are risks inherently involved when gametes/embryos are transported.
Contact a shipping company to arrange the transport. You also must make sure to meet any criteria requirements from the original clinic.
Once the gametes/embryos arrive to our facility, annual storage fees will apply with the first year payable upon receipt.

Any outstanding fees on your billing account will be settled with ONE Fertility prior to shipping.
Make sure to meet all the requirements of the new clinic prior to finalizing transport.
