POSITION TITLE: Clinic Aid

IMMEDIATE SUPERVISOR: Medical Director

EFFECTIVE DATE: April 1, 2009

JOB SUMMARY: The Clinic Aid contributes to the effective operation of ONE Fertility performing duties in every area of ONE Fertility, including lab, administration, and nursing.

POSITION TITLE: Secretary / Receptionist

IMMEDIATE SUPERVISOR: Chief Operating Officer

EFFECTIVE DATE: April 1, 2009

JOB SUMMARY:The secretary/receptionist is responsible for appointment scheduling, answering phones, greeting patients and being the front line person of ONE Fertility. The secretary/receptionist must also carry out secretarial duties as required.

DUTIES:

Serve as the initial point of contact for patients and carry out all reception related duties.

Responsible for collecting patient payments

Collate and manage the administration of patient charts

Carry out secretarial duties

Other duties as required.

POSITION TITLE:  Administrative Assistant

IMMEDIATE SUPERVISOR: Chief Operating Officer

EFFECTIVE DATE: April 1, 2009

JOB SUMMARY: The Administrative Assistant is responsible to assist with a number of accounting and clerical duties on a daily basis, including entering and maintaining accounting information, as well as assisting the receptionist.

POSITION TITLE: Registered Practical Nurse (RPN) / Phlebotomist

IMMEDIATE SUPERVISOR: Medical Director

EFFECTIVE DATE: April 1, 2009

JOB SUMMARY: The RPN contributes to the effective operation of ONE Fertility performing both direct and indirect patient care activities.  Indirect patient activities enhance the functioning of the clinic allowing it to maintain a safe and clean environment.

POSITION TITLE: Embryologist

IMMEDIATE SUPERVISOR: Scientific Director

EFFECTIVE DATE: April 1, 2009

JOB SUMMARY: The role of the Embryologist requires collaborative relationships both within and outside the laboratory.  Reporting to the Scientific Director, the Embryologist will work autonomously carrying out routine laboratory procedures in the IVF lab. This requires close collaboration with the attending REI Physician and excellent communication with clinic staff and patients.  The Embryologist will participate in establishing and maintaining standards of practice, policies and procedures at ONE Fertility. The Embryologist will perform specialized Human Assisted Reproductive Technologies (ART) at ONE Fertility while maintaining the highest level of clinical/quality control/research of embryology and andrology laboratory procedures.

POSITION TITLE:Reproductive Endocrine and Infertility (REI) Nurse

IMMEDIATE SUPERVISOR: Medical Director

EFFECTIVE DATE: April 1, 2009

JOB SUMMARY: The role of the Reproductive Endocrine (REI) Nurse requires collaborative relationships both within and outside the nursing profession.  Reporting to the Medical Director, the REI Nurse will work autonomously carrying out clinical practice and delegated medical acts. This requires close collaboration with the attending Reproductive Endocrinologist and excellent communication with clinic staff and patients.  All aspects of REI Nurse practice is based on current theory and research.  The individual will participate in establishing and maintaining standards of practice, policies and procedures at ONE Fertility.

POSITION TITLE: Andrology / Medical Laboratory Technician

IMMEDIATE SUPERVISOR: Scientific Director

EFFECTIVE DATE: April 1, 2009

JOB SUMMARY: The role of the Andrology / Medical Laboratory Technician requires collaborative relationships both within and outside the laboratory.  Reporting to the Scientific Director, the Andrology / Medical Laboratory Technician will work autonomously carrying out routine laboratory procedures in the Andrology / Blood analysis lab and will participate in establishing and maintaining standards of practice, policies and procedures at ONE Fertility. The Andrology / Medical Laboratory Technician will perform specialized diagnostic testing at ONE Fertility while maintaining the highest level of clinical/quality control/research of andrology laboratory procedures.
